Case Management
Services
The purpose of the Case Management Service is to improve the quality of life for
individuals with mental illness. To attain this goal
we employ the
principles of Psychosocial Rehabilitation with a
client centered focus. Emphasis is placed on
maximizing strengths and enhancing self esteem.
Case Management
Service Features
- Services provided by this
team are suitable for individuals requiring
moderate levels of support (2 hrs max per week)
- Case management model ie.
each client is assigned a primary worker to
co-ordinate and link services and provide
ongoing support
- Staff client ratio 1:20
- Regular appointments will
be scheduled with primary workers according to client needs
- 24 hour access to crisis
support services - 7 days per week
- Supports may be provided in
the client's home, in the community, or in the
office
